IMPORTANT INFORMATION FROM THE HEALTH OFFICE
All incoming 6th graders are required to have a Tdap and a second Varicella before the start of 6th grade. If your child has had a dose of Tdap after the age of 7, the booster dose is not required. Please check with your doctor to make sure your child is up to date. These are new immunization regulations this year! Please be sure to supply the health office with proof of these immunizations. These are mandatory vaccines and your child could be excluded from school if this requirement is not met. Without acceptable proof of required immunizations, after a 14 day “grace period”, the Principal can refuse to admit a child to school.
If your child is going to camp they will need to furnish an up to date physical and immunization record to the camp. If you need a copy of those items you must contact the school nurse before the end of the school year. The nurses do not work over the summer.
All 7th graders are required to have a physical. Please send a copy, along with any updated immunizations, to the health office as soon as possible.
Sixth graders are not eligible to play modified sports.
All 7th and 8th graders who plan to play a fall sport ( Football, Boys and Girls Soccer, Volleyball, Swimming and Cross Country) must have the sports paperwork completed and signed along with a current physical by AUGUST 10th. PLEASE DO NOT WAIT UNTIL THE LAST MINUTE! This paperwork must be reviewed by the school physician and signed off. Sometimes this process takes several days to complete. Also, I do not work in the summer so the paperwork may be in my mailbox for several days before I come in to check my mail. Please, keep this in mind.
Physicals will be offered here at the Middle School in June for current OMS students. Your child may come to the health office and sign up for a physical and pick up the sports paperwork. It’s a good idea to have a physical here, especially if your child is planning to play a fall sport and you cannot get in to your private physician until after the sport begins.
If your child has medication in the health office, please remember to pick it up before the last day of school. Medication cannot be stored over the summer. It will be discarded.
If your child will require medication, including over the counter, next school year, I will need an updated doctor note and written parent permission. The medicine must be the correct medicine in the correct bottle. Please do not send loose medication in a zip lock bag! Please do not ask me to give any medication without the required physician and parent notes.
