PHYSICALS:
The following students must have
current
physicals on file in the health
office: *7th
grade students * Students new to the district *
Students referred to the Committee of
Special Education *
Students interested in working papers *Students interested in athletics Students must stop in and get pink Health Questionnaires
and return these to the nurse with parent signatures on the pink
questionnaires. At
this time, several students have not returned
a completed physical form from their provider. These students will
therefore be
scheduled for a school physical. Please call me at 433-8260, if you
would like
to make other arrangements.

SPORTSAny students who are
interested in participating in sports should be certain to have a
current
physical on file in the health office. Physicals are
acceptable
for ONE
CALENDAR YEAR. Health questionnaires are available in the health office
for
winter sports and must be filled out for each new sports season.
