Parent Letter-Physicals

     Effective July 1, 2018 the New York State Department of Education has changed the years of mandated physical examinations from 10 grade to 9 and 11 grades. This change will require that all students in 9 and 11 grades have a physical next school year either at school or with their own provider.

      In addition to that change, State Ed is also requiring that only the approved NYS School Health Examination Form be used to document health examinations. They have made this change in order to provide consistency across the state.  This form will be available from the state in the spring and I plan to mail you a copy of it at that time. It will also be posted on the school website. A copy of this completed form must be provided to the Health Office.

       This summer please look for other forms required for the Heath Office on the High School website under Back to School Information and Forms. This will include a form that indicates your preference for a school physical or one with your student’s provider. Please print this off and have your student bring it in, along with the other forms, the first week of school.
